Arcadia Outpatient Surgery Center receives patient registration information from the referring physician.  So once you and your physician determine that a procedure is necessary, their office will call to schedule the procedure.  They will give us the time and day of procedure along with your demographic information, such as address, Date of Birth, Social Security # and insurance information.  If you want to give us any further information or have questions, please call 626-445-4714 and ask for our insurance verifier.
